- What licenses and relevant training do they hold? How can I confirm current licensure with the licensing authority?
- Who answers questions before and after the visit?
2. What exactly is being proposed?
- What is the name, purpose, and location of the proposed procedure?
- What materials or products would be used? Where do they come from, and how are they prepared?
- What regulatory status applies to this specific product and use? If this is research, what oversight and consent apply?
3. What does the evidence say?
- What published evidence supports this procedure for my situation? Can I review it?
- What is uncertain, and what risks or side effects should we discuss?
- What outcomes can reasonably be expected, and how will we measure them?
4. What choices do I have?
- What alternatives should I consider, including established approaches?
- What happens if I wait or decide against this procedure?
- Would another opinion help me understand the options?
5. What will it cost?
- Can I have a written estimate covering the evaluation, procedure, and follow-up?
- What may be billed separately? What are the deposit, financing, cancellation, and refund terms?
- What should I confirm with my insurer before agreeing to care?
6. What happens afterward?
- What follow-up is included, who provides it, and when?
- What recovery instructions and restrictions will I receive in writing?
- Who do I contact about unexpected symptoms, and what is the plan if additional care is needed?
